The Ohio Ophthalmological Society's (OOS) Play
Hard. Don't Blink. Always Wear Protective Eyewear sports eye
safety program donates free equipment which may be beneficial to
your school or local youth sports organizations.
Included with
this message is an application for your school or youth community
sports organizations to become involved with the Play Hard. Don’t
Blink. program. Please complete the form(s) and follow the
instructions for returning it to OOS. To save on postage, you may
complete the form(s) online via their website at: http://www.playhardgear.com/participate.aspx.
Please note that
the DEADLINES are November 1st for Basketball and
December 3rd for Baseball and Softball.
