Champion packet. Refer to the yellow sheet for paperwork instructions and form
download directions.

The
following forms must be downloaded, completed and e-mailed to Brenda Murray (bmurray@ohsaa.org)
by Wednesday, March 10.
Your
team photo must be e-mailed to Brenda
Murray (bmurray@ohsaa.org)
by Wednesday, March 10. If a digital photo is not available, you may mail a print
photograph to the OHSAA. It must be received by Thursday, March 12 (use overnight
mail if necessary). The digital team photo must be no smaller than 5X7 in size and
the resolution must be no less than 300 dpi or ppi. |

The

Sunday, March 14, 2010 at the Schottenstein Center, Columbus 43201.
